How do I interpret my PHQ-9 score?
Each of the nine items scores 0-3, for a total of 0-27. Roughly, 0-4 means minimal symptoms, 5-9 mild, 10-14 moderate, 15-19 moderately severe and 20 or above severe. The score is a gauge of symptom severity, not a diagnosis.
At what score should I see a doctor?
If you score 10 or higher and the symptoms have lasted more than two weeks, it is worth talking to a mental health professional. Even with a low score, seek help promptly if daily life is clearly affected or if you endorsed the last item about self-harm or thoughts of death.
How often should I take this test?
PHQ-9 asks about the past two weeks, so every two weeks is a sensible interval. If you are in treatment or counselling, logging it every 2-4 weeks helps track change. Taking it daily mostly reflects day-to-day mood swings and makes the trend harder to read.
Can this test diagnose depression?
No. The PHQ-9 is a screening tool widely used in clinical settings, but a diagnosis requires a professional interview that also weighs symptom duration, daily functioning, physical illness and medication effects. Treat the result as a useful summary to bring to a consultation.
